This could be due to a couple of things that are dependent on if you are using hand tracking or not.

If you are using hand tracking: We suggest that you grab your controllers, grab something, and then switch back to hand tracking. If this is the first time you are experiencing this we also suggest that you exit the hall and return, make sure that your physical space is well lit, your headset doesn’t need to charge, and that you are moving your hand slowly.

If you are using controllers: If you are using controller sleeves/protectors please double check that they are not touching the buttons, as even the slightest touch and affect the input. When you go to throw, be careful that no other buttons are being pushed and that your headset can see your hand as you throw.
